In 4D versions prior to 2004, you had the option to specify fields to be used when creating a new form from the explorer. In version 2004, you now have two options when creating a form from the explorer. The "+" button creates a blank form, and the button with the upside down top hat icon will initiate the form wizard, allowing you to add specific fields to the form.
