With a list form loaded in the Form Editor, you can add or delete header and break levels.

To add a header/break level, hold the alt key (on Windows) or option key (on Mac OS) and click on the header/break control marker.

To delete a header/break level, hold the ctrl key (on Windows) or command key (on Mac OS) and click on the header/break control marker.
